First identify where the failure occurred
A segment may separate through the weld, fracture within the segment, tear part of the steel core or fail after impact damage. These are different failure modes and should not be grouped together.
Evidence to collect
- Keep the blade and detached segment if available.
- Photograph the front, back, weld area and core.
- Record machine model, speed, material and cutting mode.
- Explain whether the blade was pinched, twisted or struck.
- Provide the approximate usage before failure.
Possible causes
Possible causes include insufficient weld strength, excessive side load, overheating, wrong direction, damaged flanges, unstable machine bearings, hard impact, incorrect application or an unsuitable segment/core design.
Corrective action
The supplier should inspect returned samples where possible. Corrective action may involve weld-process control, geometry changes, stronger segment retention, machine setup correction or a different product family. Safety-related failures require conservative handling and should not be solved only through text messages.
